I understand my responsibilities:	Type 'YES' if you understand and agree to the following statements: I am aware that per my lease, I am responsible for all repairs required to be made to any part of the leased premises whenever damage results from any misuse or neglect on the part of LESSEE or members of family, guests, or employees of LESSEE. I am also aware that service charges may be charged for multiple work requests within a thirty (30) day period which could have been accomplished with a single work request. I understand that if I do not authorize a release of a key to a vendor, I will need to be available to meet the vendor assigned by Browning-Gordon & Co., Inc. I also realize that scheduling appointments may create delays in service response time due to conflicting schedules between myself and the vendor. In addition, I realize if I schedule an appointment with a vendor which I do not keep, I will be responsible for the charge of the service call.
